Standard-pull Headgear

Headgear is an external appliance which is worn to correct a discrepancy in the growth between the upper and lower jaws. The standard pull headgear holds the upper jaw stationary while allowing the lower jaw to continue growing to “catch up” with the upper jaw. To produce a successful treatment result, the headgear must be worn EVERY DAY, typically a minimum of 14 hours. Many patients wear the appliance while they are:
  • Sleeping
  • Doing homework
  • Watching TV
The appliance need not be worn outside of the home. It is not critical when the appliance is worn, as long as the minimum number of hours of wear is reached. Patients may choose to wear their headgear MORE than the minimum number of hours required; doing so may shorten their treatment time. The back teeth, upon which the appliance attaches, sometimes become sensitive during the first few days of wear. This sensitivity will subside. The headgear should be brought to each appointment to be checked for proper fit. It takes time for your jaw to respond and slowly grow to the correct bite position. Headgear is one of the best ways to correct the bite and may be the ONLY way to achieve an ideal treatment result.

Reverse-pull Headgear

Reverse-pull headgear (RPHG) works in the opposite direction of the standard-pull headgear. It is used when the upper jaw is positioned behind the lower jaw. The goal of RPHG therapy is to bring the upper jaw ahead of the lower jaw. Once completed, the upper jaw will generally grow with the lower jaw. In a small percentage of patients, the lower jaw will outgrow the correction and the discrepancy between the jaws will return. In these cases, other orthodontic or orthopedic options can be explored following the completion of growth.
